Summary

The A+ T2DM Lifestyle Intervention is implemented at the Integrated Care Clinic at Haven for Hope for people with serious mental Illness (SMI). The intervention addresses unique needs of T2DM management for people with SMI and is currently offered twice a week with physical activities and group classes on health topics. OTD and DPT students have been implementing the intervention under faculty supervision.
